Time in blood pressure range and cardiovascular outcomes in HFmrEF/HFpEF: a pooled participant-level analysis of four large-scale trials

In >17,000 patients with HFmrEF/HFpEF, sustained SBP control during the first year (TIR 110–<130 mmHg) was associated with a lower subsequent risk of CV death or HF hospitalization, mainly driven by lower HF hospitalization.

  • TIR captures sustained BP control that a single office reading may miss. It carried prognostic value beyond traditional risk factors.

  • Existing medical therapies modestly increase TIR, and higher TIR is unlikely to fully explain their long-term benefit.

  • These data support the importance of longitudinal BP control in HFmrEF/HFpEF aligned with current guidelines.
